Özet :
Malign karaciğer tümörlerinin tedavisinde görüntüleme eşliğinde perkütan yolla uygulanan lokal ablasyon tekniklerinin geliştirilmesi, radyoloji alanında son 20 yılda ortaya çıkan en önemli gelişmedir. Bu teknikler arasında radyofrekans ablasyon ve mikrodalga ablasyon, etkinliği ve güvenilirliği kanıtlanmış olduğu için pek çok kurumda primer ablasyon metodları olarak kullanılmaktadır. Bu derleme primer ve metastatik karaciğer tümörlerinin görüntüleme yöntemleri kılavuzluğunda perkütan ablasyon ile tedavisi üzerine odaklanmıştır.
Sonuç :
Görüntüleme yöntemleri kılavuzluğunda perkütan yolla gerçekleştirilen lokal ablasyon prosedürleri, karaciğerin primer ve metastatik malign tümörlerinin tedavisinde son 20 yıldır başarı ile kullanılmaktadır. Özellikle RFA yönteminin çok erken ve erken evre HCC tedavisindeki klinik başarısı çok sayıda çalışma ile kanıtlanmış durumdadır. Bu çalışmalar sayesinde RFA, karaciğer tümörlerin tedavi algoritmalarında standart perkütan ablasyon yöntemi olarak yerini almıştır. Yine son yıllarda geliştirilen ve klinik kullanıma sunulan MWA yöntemi ile de, gerek cerrahi rezeksiyonla gerekse RFA ile karşılaştırılabilir düzeyde klinik sonuçlar elde edilmektedir. Yakın gelecekte perkütan ablasyonun sınırlı primer veya metastatik karaciğer tümörlerinin tedavisinde cerrahi rezeksiyonla eş konuma gelmesi kaçınılmazdır.
